NCQA Certification for Patient-Centered Medical Home

We received NCQA Certification as a Level 3 Patient-Centered Medical Home on July 9, 2009. What that means for our patients is the following:

1. Improved Access by visit, by email and by phone.

2. Partnership in your care by being prepared for your visit such as having your list of questions and concerns ready, bringing your medication list, providing your medical history including completed labs and screenings and asking questions.

3. Improved Communication between your provider and all of your medical team such as specialists, pharmacies, hospitals and other services.

4. Improved Health because our goal is to prevent disease not just treat disease through self management programs like Healthier Living Colorado.

5. Pam Hohnstein is our Patient Care Coordinator who assists your provider in coordinating your care with all of your medical neighborhood. Please feel free to contact her or myself if you have any questions about what is should mean to you to be part of a Patient-Centered Medical Home (PCMH).
